| FORMS for SUBMISSION | DESCRIPTION |

| Supervisory Committee Approval (SCA) | Students must submit the SCA a minimum of six weeks before anticipated defence date, as indicated on the Important Dates for Conferences, Scholarships, and Theses webpage. |
| Completion of all program requirements to be eligible for participation in a graduation ceremony | Due a minimum of five weeks prior to graduation date, as per the Office of the Registrar.
Fall convocation = first weekend of November Spring convocation = last weekend of April |
| Completion of post-defence requirements for conferral date | A student's post-defence requirements must be completed prior to the Office of Graduate Studies submitting a Notice of Thesis Completion (NTC) to the Office of the Registrar.
NTC submission is not available during OGS summer and winter breaks (office closures) as indicated on the Important Dates for Conferences, Scholarships, and Theses webpage (i.e. for a December 31 conferral, post-defence requirements must be completed before Winter Break). Fall conferral = December 31 Spring conferral = May 31 |
